Wellness Jax also offers a lifestyle coaching program  based on the CDC’s proven Diabetes Prevention Program, PT2. Some insurances provide reimbursement for this program and it is expected that soon Medicare will also. The CDC estimates that 1 in 3 people is at risk for developing diabetes type 2 and don’t know it. In Jacksonville, 12% of our population has been diagnosed with diabetes already.  Jacksonville has higher obesity rates, higher nutritional deficiency rates and higher rates of deaths from diabetes with less diabetes screening than most of the state. The PT2 program has shown that a structured lifestyle change that achieves 5-7% weight loss and 150 minutes a week of physical activity reduced type 2 diabetes by 58% for all ages, and 71% in people over 60, even 10 years after the program.

Participants must meet the following criteria to enroll in Wellness Jax PreventT2 program:

  • Age 18 or older and have a body mass index greater than or equal to 24 (22, if Asian); and
  • A diagnosis of prediabetes using a blood test within the past year or a history of gestational diabetes mellitus (GDM); or
  • A score on the CDC Prediabetes Screening Test  that shows high risk for prediabetes.
